Here is a blank, free “Pain Mapping” PDF.

After years of being dismissed by doctors, I realized I would have to take matters into my own hands. This pain map can be printed; you can draw or otherwise indicate your symptoms on this PDF. This allows the doctor to see what you are explaining. You can also request your doctor photocopy your pain map or hold onto a copy for your medical records.

This document can be paired with the “Appointment Sheet Template.”
