After years of being dismissed by doctors, I realized I would have to take matters into my own hands. This appointment sheet can be brought to your doctor on its own or alongside my “Pain Mapping Template.” This sheet allows you to write out the basics that you might easily forget when you go to the doctor. This includes (but isn’t limited to) things like your medicines and allergies, other recent appointments you’ve had, your symptoms, and what you desire to get out of the appointment (tests, referrals, or anything else).

I often bring a copy for myself and my doctor. You can also request your doctor hold onto a copy for your medical records.
